  • The opportunity

    Silver Chain is currently looking for an experienced, AHPRA registered accreditation professional to join our Quality and Safety team. Reporting to the Director, Quality and Safety Unit, this role will support and guide all areas of the business to maximise client safety through our robust incident management and quality improvement processes. You will ensure that legislative, care governance and contractual requirements are met. This is a varied role, taking on investigation processes, reporting and communicating with a range of key contacts both internal and external. 

    About you

    You will have at least three years’ experience working in an acute and/or community-based quality team or related clinical governance role. You will come with experience in a similar role coordinating incident investigations, including serious incident processes such as Root Cause Analysis and Critical Supporting Frameworks such as Open Disclosure. You will be very articulate and have excellent written skills allowing you to communicate effectively to stakeholders on all levels. 

    Key Responsibilities

  • Support investigation processes, including but not limited to Root Cause Analyses, Open Disclosure and incident reporting to funders and key organisational governance bodies.
  • Advise the Director, Quality and Safety Unit of any emerging or uncontrolled risks in relation to incident management.
  • In conjunction with the Director, Quality and Safety Unit, provide guidance and advice to key leaders across Silver Chain on incident management and associated compliance activities.
  • Provide co-ordination, support, mentorship to individuals undertaking serious incident investigations.
  • Develop reports on serious incident outcomes to key organisational governance committees, funders, and external regulatory agencies.
  • Monitor and advise on incident management developments occurring in the health care and aged and community care industries.
  • Lead or participate in Quality and Safety Unit quality improvement projects.
  • Provide active participation in assigned committees.
  • Facilitate serious incident skills development, including coaching and mentoring
