  • We require a talented Lifestyle Assistant to lead and coordinate lifestyle choices and activities for residents at Royal Freemasons Prahran

    About You  

    You will be a person who can build and maintain great relationships and work well in a dedicated team. The Lifestyle Assistant works with volunteers as well as work closely with therapy, and care staff providing support and activities for the residents of the home. Guided by the Lifestyle Coordinator you will engage with residents to build meaningful relationships with individualised programs that suit each resident.

    The successful candidate will bring energy and enthusiasm into this role, will be service oriented, possess exceptional people skills and be passionate about providing quality aged care activities for our residents.

    Your role includes:

  • Providing accurate documentation to enable ACFI submissions
  • Lifestyle assessments and review care plans reflecting consumer’s choice
  • Engages and builds lasting relationships with local community groups
  • Understands and meets required Aged Care Quality Standards
  • Provide Lifestyle Choices through the Relationship Model of Care
  • Providing a person-centered focus in everything you do
  • Key Requirements

  • Enthusiasm, passion for aged care, energy, inquisitiveness, initiative and innovative thinking
  • Certificate IV in Community Services (Leisure and Lifestyle) or Individual Support/Aged Care
  • At least one years’ experience in a similar role within the Health Care or Aged Care environment with exposure to residents living with dementia
  • Excellent interpersonal communication and clear written and spoken English
  • A passion for working in Aged Care and Lifestyle
  • Ability to work with a team and excellent customer services skills
  • Computer skills and ability to use Microsoft office
